If you would like your child to attend Park Road Nursery, please complete this confidential pre-registration form with details of contact numbers, doctor's name, allergies or health problems (if any) ready for when your child starts nursery. This form then enables us to contact you nearer your childs start date, to arrange sessions available.

PARENT'S AGREEMENT
Once your child is ready to start with us we need you to sign that you have read and understood the Nursery's main policies.

ARRIVAL AND COLLECTION OF YOUR CHILD
Please make sure your child arrives in good time and is collected promptly at the end of the morning or afternoon. It is very worrying to a child if you are late. Staff should be informed of any change in the collection arrangements of your child. To help your child settle into the Nursery we need a familiar adult to stay with the child initially. You are more than welcome to stay as long as you wish to help settle your child.
IF YOUR CHILD IS ILL
Children should not attend nursery for 48 hours after sickness or diarrhoea. Please call to let us know if your child is unwell.
FOOD & TOYS
Sweets, crisps and toys should not be brought to nursery.
CLOTHES
To avoid confusion, please will you clearly write your child's name on their coats.
